Space 1: Title of This Work: the headline of your home page: exactly as displayed there. Leave the other three lines blank.
Space 2: Name of Author: just fill in space 2A unless you're sharing the authorship with someone. Type your name here (sign only in space 8.) You must enter your real full name, or, if you're using a pen-name (pseudonym), you can

Dates of Birth and Death: not required but useful as a means of identification (if there are two Hartwell Hamptons in the U.S., for instance.) Birth date: enter the year. You aren't dead yet so definitely leave that space blank.
Was this a work for hire? No. Author's Nationality: enter U.S. on the 'Citizen of' line. Was this work anonymous? No (we presume.) Pseudonymous? No, unless you entered a pen-name.
Nature of authorship: "entire text; all photos; all drawings; composing an on-line work - a web site".
Space 3A: Year of Creation: whenever your site first went on the air. Or, this year, if the site isn't up yet.
Space 3B: if you've decided that your site is published, then enter month, day, and year of the publication, and "U.S." on the line below. Otherwise, leave it blank.
Space 4: Claimant: your full name and address. Transfer: leave blank.
And on to the back of the form...
Space 5: Previous registration: No.
Space 6: Since this is the first registration, leave 6A & 6B blank.
Space 7A: Deposit Account: Leave blank. If you intend to register many copyrights it might be worth your while to set up a deposit account with the Office; but we needn't get into that.
Space 7B: Correspondence: Your name, address, daytime telephone number, Fax number (if applicable) and e-mail address, or those of your agent or lawyer. If your business address isn't the same as your home address, you could enter it here. You must enter something, though.
Space 8: Certification: Let's keep it simple: you are the author so check that box; type in your name and the date (month, day, year) below; then sign it in black ink on the line marked X.
Space 9: "Certificate will be mailed ...": wherever you want the certificate to be sent. Probably to you at your home address: you should always keep the original. Your lawyer and/or agent can make do with a copy.
That's the form. That wasn't so bad, was it?
